I understand PSI activities may include sports performance training, strength training, conditioning, games, field trips, and related physical activities that involve inherent risk of injury or illness. I confirm my student is physically able to participate and authorize emergency medical care if I cannot be reached.

To the fullest extent permitted by law, I assume these risks and release Phoenix Sports Institute, Inferno Performance, and their owners, coaches, staff, volunteers, contractors, affiliates, and representatives from claims or damages arising from participation, except where prohibited by law.

I understand my selected PSI program requires a five-month semester commitment, or the remaining semester balance if enrolling after the semester begins.

A 3% card processing fee applies to online card payments. Tuition reserves a limited seat, missed scheduled program days do not reduce tuition, and payments are non-refundable except where required by law.

I authorize PSI to photograph, film, and record my student during program activities and use their image, likeness, voice, general participation, and non-confidential achievements for website, social media, advertising, print, and promotional purposes.

PSI will not publish confidential records, payment details, addresses, phone numbers, or sensitive family information without additional permission. I certify that I am the parent/legal guardian authorized to enroll this student and that the information I provide is accurate.
